Appointments
We are accepting new patients. Same day and emergency appointments are available.  Request an appointment through our website or call our office (615) 220-8788 to make an appointment for either location.  If, for some reason, you cannot keep a scheduled appointment or will be delayed, please call as soon as possible. We appreciate at least a 24 hour notice to cancel or change an appointment.

Insurance
Our office accepts a variety of HMOs, PPOs, and other health plans including, but not limited to:

Payment Options
Our office accepts Cash, Checks, Visa, Mastercard and CareCredit as payment for services not covered by insurance.  It is the patient's responsibility to provide payment for services rendered at the time of the visit that are not covered by your insurance plan.
